Policies and Procedures

All forms must be completed before scheduling an appointment. 

All patients are required to set up a patient portal.

 

Refill requests and responses from the provider must be submitted 72 business hours in advance. Please take into consideration the days that your provider is working. Business hours are Monday through Friday 9AM to 5PM (lunch from 12-1, this means that phones are also off during this time). 

 

We do not accept any pharmacy requests, such as refills. Therefore, please send refill request via patient portal (preferred), email, or phone call (if on the other line please leave voicemail). Due to the high frequency of phone calls, if a voicemail is not left, the office will not be able to return your missed call.

 

In order to provide refills for the client, the client will need to keep their appointments. No refills will be provided if the client hasn’t seen the provider in over 3 to 4 months, unless provider is aware.

 

All clients will be responsible for their co-payment on the day of their appointment. 

Please note there is no crisis line, therefore if there is an emergency please call 911, go to the nearest hospital, or call 211 for mental health services after business hours.

 

Cancellation policy for medication managment: Clients not showing at the time or canceling appointments without giving 24 business hour notice (Monday through Friday) will be charged a $75.00 no-show fee.  If a client cancels anytime between Friday at 5pm and Monday at 9AM, it will be considered a no-show, as those hours are outside of business hours.

 

Cancellation policy for therapy and a new patient evaluationsL.  Clients canceling appointments without giving 48 business hour notice (Monday through Friday) will be charged a $125.00 no-show fee. If a client cancels anytime between Friday at 5pm and Monday at 9AM, it will be considered a no-show, as those hours are outside of business hours.

 

If the client has 2 no shows back to back, cancels a numerous amount of times within a short period, is non-compliant, or is disrespectful to the staff, the provider is able to discharge you from treatment. Any no shows exceeding two can result in termination of services.  

 

All clients will need to know their deductible amount before scheduling an appointment. This will eliminate the client from questions and  errors/misleading/unknown information regarding their deductible and amount owed for their visit.  Please note, if you have a deductible plan, you are responsible for the deductible amount due that the insurance company states, before the deductible is met.  If you are concerned about the amount of the deductible due at the time of visit, you may contact us to get the billing code for you to provide to your insurance plan. Updated insurance will be required by our office within 30 days of the active start date.

 

Please note, that providers sometimes run behind after their appointments, therefore please allow yourself extra time before & after the visit, in case this is these circumstance. 

 

All patients are responsible for calling the office to make a follow up appointment if it is not done with the provider at the time of the appointment. Please contact the front office only via phone or email to schedule an appointment, this should not be done via the patient portal.

 

When scheduling a telehealth (virtual) appointment,  you will receive an email with a link. Please click the link at the time of the appointment.  These links can also be found on our website https://www.generationspsychiatry.com

 

1 week notice is required for all paperwork to be completed by the provider and the patient may be charged for their time and effort taken to complete this paperwork.

 

Clients are responsible for finding out what other pharmacies in their area have their medication, if their medication is out of stock at their pharmacy.

 

We try to provide a healthy work/life balance for our providers, therefore these policies are important to us and are required to be implemented into our practice, this will ensure that all our providers can implement the best care they can to our valued patients!
